The SELECT Trial: A Cautionary Tale
The Selenium and Vitamin E Cancer Prevention Trial (SELECT) was a landmark NIH study of 35,533 men designed to test whether selenium and/or vitamin E supplementation prevented prostate cancer. The results were devastating for supplement marketing: selenium supplementation showed no cancer prevention benefit and was associated with a non-significant trend toward increased type 2 diabetes risk. Vitamin E supplementation was associated with a 17% increase in prostate cancer.
SELECT used 200 mcg/day of L-selenomethionine in a population that was largely selenium-sufficient at baseline. This is critical context: selenium supplementation in already-sufficient populations may not only be useless but potentially harmful. The U-shaped dose-response curve for selenium — where both deficiency and excess cause problems — is one of the most well-established findings in micronutrient research and one of the most ignored by supplement companies.
Forms of Supplemental Selenium
- Selenomethionine: Organic form incorporated into body proteins. Higher bioavailability, longer tissue retention. Most commonly used in studies.
- Sodium selenite: Inorganic form. Used in the EUGOGO thyroid eye disease trial. Less tissue accumulation (potentially safer for long-term use).
- Selenium-enriched yeast: Contains mainly selenomethionine plus other selenocompounds. Variable composition between brands.
- Methylselenocysteine: Studied in cancer research. Less commercial availability.
Toxicity: Selenosis
Chronic selenium intake above 400 mcg/day can produce selenosis, characterized by:

- Men seeking cancer prevention — SELECT showed no benefit and possible harm
- Anyone already taking a multivitamin containing selenium plus eating a selenium-rich diet (Brazil nuts, seafood, organ meats) — total intake may exceed safe limits
- Individuals with type 2 diabetes risk factors — SELECT suggested possible diabetes risk increase
