The safest starting point is simple: do not compare promises first. Compare the full label, your current medicines, and the total amount of each ingredient you already use. A performance supplement may contain caffeine, creatine, amino acids, herbs, vitamins, minerals, or several of these at once. The more ingredients and products you combine, the harder it becomes to predict side effects.
Start With the Supplement Facts Label
Use the Supplement Facts panel, not the front-label promises, as your first checkpoint. The FDA explains that this panel should list serving size, servings per container, dietary ingredients, and the amount of each ingredient per serving. A supplement is not approved by the FDA for safety or effectiveness before it reaches stores, so a label is a starting point for evaluation, not proof that a product is right for you. The NIH Office of Dietary Supplements notes that many performance products combine ingredients in varying amounts, while research often studies only one ingredient at a time. That means evidence about an ingredient does not automatically apply to a specific blend. Check the Total Dose Across Your Whole Routine
“Stacking” means using two or more products at the same time. It can happen by accident. For example, a pre-workout powder, fat-loss capsule, coffee, electrolyte drink, and daily vitamin may overlap in stimulants, vitamins, minerals, or herbs.
Check for repeated ingredients before adding anything new. Caffeine and stimulant-type ingredients deserve special attention because several sources can add up quickly. High amounts of vitamins and minerals can also create problems even when each individual product appears modest.
Some ingredient evidence is limited or mixed. The NIH consumer guide reports little clear evidence that several commonly promoted ingredients, including arginine, reliably improve strength or aerobic performance. Use studied amounts as context, not a personal dose:
- Caffeine: NIH describes 2 to 6 milligrams per kilogram of body weight as the usual performance range used in research. That is about 210 to 420 milligrams for a 154-pound adult. More is not shown to improve performance further and can raise the chance of side effects.
- Creatine monohydrate: Studies often use about 20 grams per day, split into four servings, for 5 to 7 days, followed by 3 to 5 grams per day. Water-weight gain and stomach upset can occur, and this study pattern does not make creatine appropriate for every reader.
- Multi-ingredient blends: There is no single useful comparison amount when the exact dose of each ingredient is hidden or when the finished combination has not been studied.
A practical rule: add only one new supplement at a time, keep a dated list of what you take, and do not increase the dose just because early results feel uncertain. Bring a list of all supplements and medications you take, including the dose and how often you take each one, when you speak with a pharmacist or clinician.
Medication, Surgery, and Pregnancy Checks Come First
Supplements can change how your body absorbs, breaks down, or removes a medicine. This can leave you with too much or too little medication in your system. The FDA warns that some supplements can also interfere with lab tests or create risks around surgery.
Talk with a qualified clinician or pharmacist before using a fitness or performance supplement if you take medication for heart disease, blood pressure, mood, blood clotting, diabetes, HIV, transplant care, or birth control. - Before surgery: Tell your surgical team about every supplement. They may ask you to stop some products two or three weeks before a procedure because of possible effects on bleeding, heart rate, or blood pressure.

- Children and teens: Do not assume an adult fitness product is appropriate. Safety data can be limited, and body size and metabolism differ.
Know When Health Conditions Raise the Stakes
“Natural” does not mean risk-free. A person with kidney disease, liver disease, heart disease, uncontrolled blood pressure, a bleeding disorder, or a history of serious mental-health symptoms should get personal guidance before using a performance product.
Creatine is a common example of why context matters. NCCIH says it may somewhat enhance the effects of exercise on strength, muscle mass, and endurance, but short-term side effects can occur and long-term effects are not well studied. NCCIH also advises people at risk of kidney problems to check with a health care provider before use. Do not use a supplement as a substitute for prescribed treatment, a nutritious diet, adequate hydration, or proper training. Those basics are the foundation of exercise performance; a supplement cannot reliably make up for their absence.
Watch for Contamination and Hormone-Style Red Flags
Contamination means a product contains an ingredient that is not on the label, or contains an ingredient at a different amount than expected. This is especially important for products marketed as extreme muscle builders, hormone boosters, steroid alternatives, or rapid body-composition solutions.
The FDA warns that some bodybuilding products illegally contain anabolic steroids or steroid-like substances. These are hormone-related compounds associated with serious risks, including liver injury, kidney damage, heart attack, stroke, blood clots, mood changes, and sexual side effects. Pause before using a product when you see:
- Claims of dramatic muscle gain, hormone changes, or “legal steroid” effects.
- Instructions to stack several products from the same seller.
- An incomplete label, an undisclosed blend, or unclear manufacturer contact details.
- Claims that a product treats disease or replaces medical care.
- Directions to take more than the label states, dismiss a bad reaction, or avoid telling your clinician what supplements you use.
Stop Use and Seek Care for Serious Symptoms
If you think a supplement is causing a bad reaction, stop taking it and seek medical advice. The FDA calls a bad reaction an adverse event. Prompt action matters because symptoms can be easier to assess when you can identify the product, dose, timing, and other medicines or supplements involved.
Seek urgent care for chest pain, shortness of breath, fainting, severe allergic symptoms, sudden weakness, confusion, or signs of a possible stroke. Contact a clinician promptly for severe nausea, unusual fatigue, fever, abdominal pain, yellow skin or eyes, or dark or discolored urine—especially after using a bodybuilding or hormone-style product.
Keep the container, take photos of the label, and note when symptoms began. The FDA encourages consumers and clinicians to report adverse events. Reporting does not prove a product caused the event, but it can help identify safety patterns.
Use a Safer Comparison Process
When comparing products, review complete ingredient amounts, duplicate ingredients, medication and condition cautions, warning signs, and the evidence for the stated purpose. Be wary of advertising hype and claims that sound too good to be true. Evidence about one ingredient does not establish the safety or effectiveness of an unstudied finished blend.
