Anti-Aging and Longevity Supplements: Cellular Health and Evidence
The anti-aging supplement market is a multi-billion dollar industry promising youth, vitality, and extended lifespan through pills and powders. Products containing resveratrol, NMN, rapamycin analogs, and exotic polyphenols market directly to aging-anxious consumers. Yet the evidence for human longevity effects is virtually nonexistent. Cost is prohibitive ($100-400 monthly); benefits are marginal and temporary (effects cease upon discontinuation). Realistic assessment: NAD+ precursors have plausible biological rationale but unproven human benefit for aging or longevity.
Polyphenols and SIRT1 Activation: Resveratrol, Quercetin, and Related Compounds
Polyphenols activate SIRT1 through NAD+ metabolism. Resveratrol (from red grapes and red wine) is the most-marketed polyphenol claiming anti-aging effects. However, human evidence is remarkably weak:
Resveratrol: Meta-analyses show no consistent benefit for cardiovascular disease, diabetes, cancer, or cognitive decline in humans. One study followed healthy adults taking resveratrol 1,000+ mg/day for 2 years and found no difference in aging markers compared to placebo. The compound is enthusiastically promoted; the evidence is disappointing.
Quercetin, catechins, other polyphenols: Similar pattern: plausible mechanisms in animal models, weak or absent human evidence. Epidemiologic studies show polyphenol-rich diets (berries, tea, dark chocolate, cruciferous vegetables) correlate with healthspan and longevity, but isolated supplementation has not replicated these benefits.
This suggests the benefit is not from isolated polyphenols but from the dietary pattern and other lifestyle factors in populations consuming these foods.
Metformin and Senolytics: Emerging “Anti-Aging” Agents (Not Yet Supplementary)
Two agents with preclinical and early clinical evidence for aging effects deserve mention:
Metformin: The diabetes medication has epidemiologic evidence suggesting diabetic patients on metformin have lower mortality and potentially extended lifespan compared to diabetics on other medications. Some hypothesis that metformin activates AMPK (an aging-regulatory pathway). However, metformin is a pharmaceutical drug, not a supplement. Use in non-diabetic people for life extension is off-label and not recommended without medical supervision. Studies in non-diabetic people are minimal.
Senolytics: Experimental compounds that remove senescent cells (non-functional aging cells accumulating with age). Dasatinib and quercetin together show senolytic effects in mouse models. Human senolytic data is preliminary. These are not currently available as supplements.
Practical Anti-Aging Strategy: What Actually Works

Evidence-based life extension approaches (in order of effect size):

Intermittent fasting (16:8 or 5:2 protocols) shows similar cellular benefits. This is the strongest evidence-based aging intervention available.
- Physical exercise: Regular aerobic exercise and resistance training increase mitochondrial biogenesis, activate AMPK/SIRT1 pathways, reduce inflammation, and support metabolic health. Exercise is one of the most consistent predictors of healthy aging and extended lifespan in longitudinal studies.
- Sleep optimization: Sleep is when mitochondrial biogenesis and cellular repair occur. Consistent 7-9 hours nightly predicts longevity. Poor sleep accelerates aging through multiple mechanisms.
- Stress reduction: Chronic psychological stress accelerates aging through telomere attrition, increased inflammation, and mitochondrial dysfunction. Meditation, yoga, and social connection support healthy aging.
- Polyphenol-rich diet: Mediterranean and plant-based diets rich in vegetables, berries, tea, and whole grains correlate with healthspan and longevity. This is the strongest dietary evidence. Isolated polyphenol supplementation has not replicated these benefits.
- Cardiovascular health: Hypertension, dyslipidemia, and atherosclerosis are strong aging accelerators. Treating cardiovascular risk factors has proven lifespan extension benefits.
Supplements with modest evidence for healthy aging:
- Vitamin D (1,000-2,000 IU/day for bone health, immune function, and mood)
- Omega-3 fatty acids (1-2g EPA/DHA daily for cardiovascular and cognitive health)
- Polyphenol-rich foods (not isolated supplements): berries, tea, dark chocolate, cruciferous vegetables
Supplements marketed for anti-aging but with weak evidence:
- Resveratrol (plausible mechanism; no proven human benefit)
- NAD+ precursors (modest mitochondrial effects; longevity unknown)
- Quercetin, catechins (plausible; weak human evidence)
- Rapamycin analogs (prescription immunosuppressants; human anti-aging data absent)
The Business of Anti-Aging Supplements: Marketing vs. Science
The anti-aging supplement industry exploits a fundamental human desire: to slow aging and extend life. Yet most anti-aging products are sold on theoretical mechanisms and animal data rather than human clinical evidence. When human trials are conducted, effects are typically modest or absent.
The financial incentive is clear: selling supplements targeting aging-anxious consumers is lucrative. The scientific evidence is weaker than marketing suggests.
Realistic Anti-Aging Supplementation Strategy

- Prioritize behavioral interventions. Caloric restriction (mild, 10-20% deficit), regular exercise, consistent sleep, and stress management have far stronger evidence for healthy aging than any supplement.
- Dietary patterns matter more than isolated supplements. Mediterranean and plant-based diets rich in whole foods predict longevity. Isolated polyphenol supplements do not replicate these benefits.
- Support general health with targeted supplementation. Vitamin D (1,000-2,000 IU/day), omega-3 (1-2g/day), and micronutrients (adequate iron, B12, magnesium) support healthy aging indirectly by maintaining cardiovascular, immune, and mitochondrial health.
- Be skeptical of “longevity supplement” marketing. No human supplement has been proven to extend lifespan. Claims of anti-aging benefits in marketing copy are unfounded. Scientific evidence must precede medical claims.
- Consider NAD+ precursors cautiously. If interested in NMN or NR for mitochondrial health (reasonable biological rationale), supplements are expensive and effects are modest. Cost-benefit is poor for most people.
